This Billing & Financial Policy applies to medical services provided by affiliated professional medical entities, including:
- HLTH Lab Medical LLC
- Doctors Office & Walk-In Clinic CA, Inc.
- Doctors Office CT PLLC
Ozara (HLTH LAB Inc.) provides technology and administrative support services only and does not bill for professional medical services unless explicitly stated.
By scheduling or receiving services, you agree to the terms below.
1. Insurance Billing
If you provide insurance information:
- Claims will be submitted to your insurance carrier when applicable.
- Coverage depends on your specific plan and benefits.
- Verification of benefits is not a guarantee of payment.
You are responsible for:
- Copayments
- Deductibles
- Coinsurance
- Non-covered services
- Services denied by your insurer
If your insurance denies payment, you are financially responsible for the balance.
2. Self-Pay Patients
If you do not provide insurance or choose not to use insurance:
- Payment is due at the time of service.
- Fees will be disclosed prior to service when possible.
3. Telehealth & Asynchronous Visits
Telehealth and asynchronous services may be billed as medical visits when clinically appropriate.
Response times for asynchronous services may vary.
Charges may apply even if:
- A prescription is not issued
- Testing is not ordered
- Referral is not made
You are paying for professional evaluation and clinical judgment.
4. Copayments & Deductibles
Copayments and known patient responsibility amounts may be collected:
- At the time of scheduling
- At the time of service
- After claim processing
If additional patient responsibility remains after insurance processes the claim, you will receive a statement.
Payment is due within 30 days of statement date.
5. Returned Payments & Chargebacks
If a payment method is declined or reversed:
- You remain responsible for the balance.
- A processing fee may apply.
Fraudulent chargebacks may result in suspension of services.
6. Membership or Subscription Services
If you enroll in a membership or subscription program through HLTH LAB Inc.:
- Fees may be billed separately from medical services.
- Membership fees may not be covered by insurance.
- Recurring charges will continue until canceled.
Cancellation terms will be provided at enrollment.
7. Out-of-Network Services
If services are provided out-of-network:
- You may be responsible for full payment at time of service.
- Documentation may be provided for you to submit to your insurer.
- Reimbursement is not guaranteed.
8. Refund Policy
Payments for completed medical services are generally non-refundable.
Refunds may be considered in limited circumstances, including:
- Duplicate payments
- Billing errors
Refund determinations are made at the discretion of the applicable professional medical entity.
9. Collections
Accounts with unpaid balances may be:
- Subject to collection efforts
- Referred to a collection agency
You are responsible for reasonable collection costs where permitted by law.
10. Financial Hardship
If you experience financial hardship, contact our billing department to discuss available options.
