Ozara (DBA of HLTH LAB Inc.)

1. Corporate Structure & Roles

Ozara is a brand name and DBA of HLTH LAB Inc., a technology and management services organization (MSO).

Medical services are delivered by affiliated, independently organized professional entities, including:

  • HLTH Lab Medical LLC
  • Doctors Office & Walk-In Clinic CA, Inc.
  • Doctors Office CT PLLC

These professional entities:

  • Employ or contract with licensed clinicians
  • Deliver medical services
  • Maintain clinical control and medical decision-making authority
  • Bill and collect for professional medical services
  • Contract with payors and insurers

HLTH LAB Inc. (Ozara):

  • Provides technology infrastructure
  • Provides administrative and management services
  • Supports scheduling, billing support, intake systems, and patient communications
  • Does not practice medicine
  • Does not control clinical decision-making

All clinical decisions are made exclusively by licensed healthcare professionals employed or contracted by the applicable professional medical entity.

2. Scope of Medical Services

Medical services may include, depending on state and provider availability:

  • Preventive and primary care services
  • Chronic disease management
  • Virtual and in-person consultations
  • Diagnostic evaluation
  • Care coordination
  • Health optimization services

Services vary by state due to licensing laws and regulatory requirements.

Care is provided only where clinicians are properly licensed.

3. Insurance Participation

The professional medical entities may participate with commercial insurance plans, including but not limited to:

  • Aetna
  • Cigna
  • Blue Cross Blue Shield (varies by state)
  • UnitedHealthcare/Oxford
  • ConnectiCare (CT)
  • MultiPlan

Participation is entity-specific and state-specific. Insurance acceptance is subject to:

  • Network status
  • Plan type (HMO, PPO, EPO, POS, etc.)
  • Employer group structure
  • Referral requirements
  • Prior authorization requirements

Verification of benefits is not a guarantee of payment.

4. Billing & Financial Responsibility

4.1 Professional Services Billing

Medical services are billed by the applicable professional entity providing care.

Patients are financially responsible for:

  • Copayments
  • Deductibles
  • Coinsurance
  • Non-covered services
  • Services denied by the insurer

4.2 Technology or Administrative Services

Certain non-clinical services may be provided through HLTH LAB Inc. under separate terms, if applicable.

These may include:

  • Membership programs
  • Digital health platform access
  • Administrative support services

These services may not be covered by insurance.

5. Prior Authorizations & Referrals

Some services require:

  • Primary care referral
  • Insurance pre-authorization
  • Medical necessity review

Patients are responsible for ensuring referral compliance when required by their plan.

Failure to obtain required authorization may result in patient financial responsibility.

6. Out-of-Network Services

If a professional entity is out-of-network with a patient’s plan:

  • Services may still be provided
  • Patients may be responsible for full payment
  • Documentation may be provided for patient submission to insurer

Payment responsibility remains with the patient unless insurer reimbursement is received.

7. Clinical Independence & Medical Judgment

All medical decisions are made solely by licensed clinicians.

HLTH LAB Inc. does not:

  • Interfere with clinical decision-making
  • Direct diagnoses or treatment plans
  • Influence medical necessity determinations

Professional entities retain exclusive authority over:

  • Hiring clinicians
  • Supervising clinical staff
  • Establishing medical protocols
  • Determining standards of care

8. No Corporate Practice of Medicine

HLTH LAB Inc. operates in compliance with state corporate practice of medicine laws.

Medical services are provided exclusively by duly formed professional entities authorized to practice medicine in their respective states.

Nothing on this website constitutes the practice of medicine by HLTH LAB Inc.

9. Patient Acknowledgment

By receiving services, patients acknowledge:

  • They are receiving medical care from the applicable professional medical entity
  • HLTH LAB Inc. provides administrative and technology support only
  • Insurance coverage is subject to plan terms
  • They remain financially responsible for non-covered charges

10. Regulatory Compliance

Professional entities and affiliated providers comply with:

  • Applicable state medical board regulations
  • Federal healthcare laws
  • HIPAA privacy regulations
  • Insurance contract requirements

Care is delivered in accordance with all applicable legal standards.
